Embryo Banking Package

➢ This package includes:

Customized Cycle Planning & Protocol Preparation by Dedicated IVF Care Coordinator
Direct consultation with Physician during cycle
Ultrasound guided oocyte retrieval
Oocyte identification, sperm preparation, insemination (ICSI).
Blastocyst culture using EmbryoScope™+
Blastocyst assisted hatching
Blastocyst cryopreservation and 1 year cryostorage.

➢ Services EXCLUDED from package:

Payment is due in FULL before treatment begins.
Maximum number of embryos stored is 15, each additional embryo frozen is $350.
The included cryostorage year begins on the retrieval date; storage beyond this period will incur additional fees (see Cryostorage Fee Schedule).
Once paid, package is valid for 1 calendar year, treatment must start by 1 year anniversary of payment.
EXCLUDED from refunds based on performance.
Patient may upgrade to the Unlimited program, by the end of the 2nd Embryo Banking cycle.
For the Unlimited Program, year starts at 1st Egg Retrieval procedure.

Payment is due in FULL before treatment begins.
If no oocytes are retrieved, package is preserved with a Procedure Fee ($950). Procedure Fee cannot be utilized, if number of oocytes retrieved, does not meet expectations.
Maximum number of oocytes covered in package is 15. Additional oocytes frozen will incur an additional fee at $250 per oocyte.
The included cryostorage year begins on the retrieval date; storage beyond this period will incur additional fees (see Cryostorage Fee Schedule).
Once paid, package is valid for 1 calendar year, treatment must start by 1 year anniversary of payment.

Payment is due in FULL before treatment begins.
One can choose how many oocytes but total number is limited by how eggs are frozen. Each new thawing cycle requires the purchase of a new package.
Maximum number of oocytes thawed in package is 25. Additional oocytes thawed will incur an additional fee of $1000 per 10 oocytes.
The included cryostorage year begins on the thaw date; storage beyond this period will incur additional fees (see Cryostorage Fee Schedule).
Once paid, package is valid for 1 calendar year, treatment must start by 1 year anniversary of payment.
No guarantee of oocyte survival of thawing & EXCLUDED from refunds based on performance.
